Job Description: Care Co-ordinator
Are you a compassionate, organised, and proactive individual with experience in domiciliary care? We are looking for a dedicated Care Coordinator to join our growing team and play a key role in delivering high-quality home care services.
Key Responsibilities:
- Coordinate and schedule care visits in line with client needs and staff availability
- Conduct risk assessments, care needs assessments, and prepare personalised care plans
- Liaise with clients, families, carers, and external professionals
- Ensure compliance with CQC standards and internal policies
- Maintain accurate and up-to-date care records and staff rotas
- Support recruitment, induction, and supervision of care staff
- Provide on-call support on a rota basis
Requirements:
- Previous experience in domiciliary care or a similar care coordination role
- Strong organisational and communication skills
- Ability to work under pressure and meet deadlines
- Good IT skills and familiarity with care management systems
- Driving licence and access to own vehicle preferred
- NVQ Level 3 in Health & Social Care (or working towards) desirable
